Abstract

The article presents the results of a study that help to understand how widespread the consumption of fee-for-service medicine is, and what the amounts of the official and unofficial payment for medical care, medicines and medical supplies in today’s Russia are. The analysis relies on the data of Russia Longitudinal Monitoring Survey – Higher School of Economics (RLMS–HSE). The study showed that Russians are increasingly being forced to turn to paid medical services in private and public medical institutions, and spend considerable funds for purchasing medicines and medical supplies, not only for the prevention of diseases and treatment at home, but also for treatment in polyclinics and hospitals. At the same time, the level of legal payments is growing and the segment of shadow payment for medical services is shrinking. The findings complement the analysis of inequalities in access to health care, provision of medicines and medical supplies between individual socio-demographic groups. Significant differences in the frequency and amount of payment for medical services are revealed, which are caused by the inequality among Russians regarding the level of income security. It is emphasized that the most significantly limited opportunities of access to paid medicine services are among people in difficult situations (poor, disabled, unemployed, caring for sick family members with a large number of dependents, etc.), who constantly experience acute deprivation in meeting basic needs due to lack of funds. The low standard of living of vulnerable social groups and the underdeveloped social guarantees system, which do not provide equal access to quality paid medical care, are becoming important reasons for the deterioration in health indicators of the population.
